Individual track PCM requests

Just found the best ever version of Frog's Theme for Chrono Trigger? Just made the most awesome remix of Guile's Theme for Street Fighter II? Your kid compose some music, and you want to surprise them with it playing in their favorite SNES game? This is the place to be!

I figure it's far better to keep all such requests centralized to one place, rather than having requests popping up randomly in threads all over the place. This also makes it more likely someone with the skills to convert the music will actually see the request.
